Registration Forms for Fully Covered/Rent-Controlled Units
| Form | Use |
|---|---|
| Vacancy Registration Form (VR) for Tenancies Starting ON or AFTER January 1, 1999 | Must be filed within 15 days after the start of a completely new tenancy, or after the last original occupants have departed. |
| VR for Tenancies Starting PRIOR to January 1, 1999 | For pre-1999 tenancies not previously reported to the Rent Board. |
| Amended Registration Statement (ARS) | Report a change of ownership, mailing address, or status of a previously registered unit. |
| Initial Registration Statement (IRS) | For units that have NEVER been registered with the Rent Board. If you are using this form to register a rental unit, you must ALSO file a VR. |
| Living Trust Declaration for Owner-Occupancy Exemption | Must be filed with an ARS when claiming an exemption of “owner-occupancy” for properties that are owned as a revocable living trust. (Rent Board Regulation 503(B).) |
Registration Forms for Partially Covered Units
| Form | Use |

| Registration Statement for Partially Covered Units | To register a partially covered unit that has never been registered with the Rent Board, change a unit's status, claim an exemption, or report a change of ownership/mailing address. If you are using this form to register a partially covered unit for the first time, you must ALSO file a Tenancy Registration Statement.
